This is from a wonderful compilation album of modernized hymns called "Jesus, Firm Foundation". I highly recommend the album. I put a pretty acoustic guitar piece by Richard Bennett at the beginning, called "Fields Of Plenty". Anyway this is a classic hymn originally written as a poem in 1752. The music was added in 1899; the tune is Finlandia.
